You're Losing Weight On Peptides — But Is It The Right Kind?
GLP-1 medications & weight-loss peptides are everywhere right now. Ozempic. Wegovy. BCP-157. Retatrutide. Someone in your family is probably on one. Someone at your workplace. Maybe you are. And depending on who you talk to, people either think they are the greatest thing to happen to weight loss or a shortcut that should not exist.
I am not here to take either of those positions. Although I have my position on it.
What I am here to do is have the conversation that most fitness professionals are either too uncomfortable or too uninformed to have. Because whether you are on a GLP-1 right now, considering it, or just trying to understand what all the noise is about — there are things happening inside the body on these medications that almost nobody is talking about. And I think you deserve to actually know them.
No judgment. Just the full picture.
The Results Are Real. So Is The Problem Nobody Mentions.
GLP-1 medications work. I am not going to pretend otherwise. They suppress appetite significantly, they help people lose weight, and for some people that weight loss carries real health benefits — improved blood sugar, lower blood pressure, better mobility. That is not nothing and I am not dismissing it.
But here is where I need you to slow down and pay attention.
The weight is coming off fast. And when weight comes off fast — especially when it comes off because you are barely eating — it is not just fat you are losing.
You are losing muscle.
Most people on these medications are not tracking their body composition. They are stepping on a scale, watching the number drop, and calling it working. And by one very limited measure, it is. But the scale does not tell you what you are actually losing. It does not tell you the ratio of fat to muscle. It does not tell you what is happening to your bone density. It just shows you a smaller number and lets you fill in the rest with hope.
Here is a question worth sitting with — do you actually know what your body composition looks like right now? Not your weight. Your muscle mass. Your fat percentage. Your bone density. Do you have any idea?
If you have access to a gym or studio, many of them have a bio-scale — sometimes for a small fee — that can give you a real breakdown of what is going on inside your body. Not just your total weight, but your muscle mass, body fat percentage, hydration, and more. That information changes the conversation entirely. Because when you can see that the scale dropping also means muscle dropping, it stops feeling like a win.
Starving Through Keto Or Peptides Is Still Starving…
This is the part that does not get said enough.
When you are on a GLP-1 and your appetite is suppressed to the point where you are eating very little, your body does not know you are on a medication. It does not understand that this is intentional. It just knows it is not getting enough fuel and it responds the same way it always has — by slowing your metabolism down to match what it is receiving, protecting fat stores because fat is long term energy, and breaking down muscle because muscle is expensive to maintain and your body will sacrifice it to survive.
This is the same metabolic adaptation that happens with extreme dieting. Same biology. Same consequences. The medication changes your hunger signals. It does not change how your body responds to starvation.
You are also quietly starving yourself of nutrition. When you are barely eating, you are barely getting the vitamins, minerals, and nutrients your body needs to function. Bone density does not decline overnight. A nutritional deficiency does not announce itself. These things build quietly underneath the surface until one day they are not quiet anymore.
By the time you feel it, it has already been happening for a while.
The Weight Loss Obsession — And Why I Can Call It Out
I want to say something that might ruffle some feathers. Chasing weight loss as your only goal is one of the most damaging things you can do for your long term health. I say that with full respect and zero judgment — because I have been there myself.
I did unhealthy things to lose weight. I chased a number. I made decisions based on what the scale said without understanding or caring about what was actually happening inside my body. I thought that was what fitness was about. A lot of people think that. The industry has spent decades teaching us to think exactly that.
And then there is BMI — a measurement that is genuinely outdated and has no business being used as a primary health marker in 2026. BMI was never designed to measure individual health. It does not account for muscle mass, bone density, body composition, age, or gender. A muscular, fit person can be classified as overweight or obese by BMI while someone with very little muscle and poor metabolic health can show up as perfectly normal. It is a blunt tool being used for a job that requires precision.
GLP-1 medications have plugged directly into the weight loss obsession. Doctors prescribe them, the scale drops fast, the BMI number goes down, and everyone calls it a success. Meanwhile nobody is asking about muscle mass. Nobody is checking in on what the person is actually eating. Nobody is monitoring bone density or long term metabolic health.
Fast results feel good. I understand that better than most. But when you look up in two or three years and your metabolism has crashed, your muscle has quietly disappeared, and the weight has come back because none of the real patterns were ever addressed — what did you actually gain?
What Your Body Actually Needs Right Now
If you are on a GLP-1 medication, here is what matters most — not instead of the medication, alongside it.
Protect your muscle. Resistance training is not optional here. Lifting weights signals your body to hold onto lean mass even in a calorie deficit. Without it, a significant portion of what you are losing is muscle you will regret losing down the road.
Make protein the priority at every single meal. Even when you are not hungry. Even when eating feels like a task. Your body needs protein to preserve muscle, support bone health, and keep your metabolism from crashing. If you are only eating small amounts, what you eat needs to count.
Stop using the scale as your only measure of progress. Body composition is what matters. Less fat, maintained or improved muscle mass, healthy bone density. A bio-scale, a DEXA scan, or even consistent progress photos and measurements will tell you far more than your weight ever will.
Eat enough. There is a difference between eating less and eating almost nothing. Your body needs fuel and nutrients to function. Suppressed appetite does not mean your nutritional needs have gone away.

How I Would Actually Help You

I would not tell you to stop the medication. I would not lecture you. I would sit down with you, look at what you are actually eating, make sure your protein is where it needs to be, get you lifting to protect your muscle, and help you understand what your body composition actually looks like — not just what the scale says.
Because here is the honest truth: the medication can help you lose weight. But it cannot teach you how to eat. It cannot build your muscle or protect your bone density. It cannot fix the patterns or the relationship with food that may have contributed to where you are. And it cannot guarantee the results last.
